For assistance or more information, please contact the Admissions Specialist, <a href=\"mpierc26@utk.edu\">Macey Pierce<\/a>. All completed applications will be reviewed.<\/p>\n<p>After the application deadline, the Application Screening Committee will schedule on-campus interviews in for individuals chosen to go on to the next step in the admissions process.<\/p>\n<h4>Application Checklist:<\/h4>\n<ul>\n<li>Student application<\/li>\n<li>Parent\/guardian Information to be completed by parent\/guardian<\/li>\n<li>Emergency Contact\/Medical Information form<\/li>\n<li>High school transcript<\/li>\n<li>Copy of applicant&#8217;s IEP or equivalent documents.<\/li>\n<li>Educational evaluation (ideally conducted within the past three years) such as:\n<ul>\n<li>Wechsler Adult Intelligence Scale<\/li>\n<\/ul>\n<\/li>\n<li>Psychological\/behavioral evaluation that includes the applicant&#8217;s Full Scale IQ and is valid within the past five years<\/li>\n<li>Results of a current physical examination (within last 12 months)<\/li>\n<li>Inventories completed by parent\/guardian<\/li>\n<li>A picture of the applicant where the face is clearly visible.<\/li>\n<li>Three letters of recommendation from persons who have known the applicant for one year or longer which can be submitted <a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSdrDCnWIBBHSdB_0ITvryP_L5vOOlzrVJ-FTx7u96hjmIf_Iw\/viewform?usp=header\">here<\/a>. The recommendations should represent three of the following areas:\n<ul>\n<li>Education<\/li>\n<li>Vocational\/employment<\/li>\n<li>Community involvement<\/li>\n<li>Personal<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p style=\"padding-left: 60px\"><strong>NOTE:<\/strong> Family members are <strong>not<\/strong> to complete letters of recommendation.<\/p>\n<p>A paper application is available upon request to mpierc26@utk.edu.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The FUTURE Program operates on a fall enrollment schedule.
