They allow students to learn more about a specific service area and to gain experiences in the field while serving the field. This memorandum is designed to provide basic information about the practicum experience (HEAM 599) and the forms to be completed in relation to the experience.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">General Information<\/h3>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Students must be accepted into the SAHE Program to enroll in HEAM 599.<\/li>\n\n\n\n<li>Students must complete a minimum of 270 hours in 2 to 3 practcium (90 hours = 1 credit hour) of HEAM 599.<\/li>\n\n\n\n<li>Students may not complete a practicum at their current place of employment (full-time position, part-time position, or graduate assistantship) without special permission from the SAHE Program Coordinator.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Practicum Forms<\/h3>\n\n\n\n<p class=\"fancyLink\"><a href=\"https:\/\/forms.office.com\/Pages\/ResponsePage.aspx?id=2RNYUX1x3UWeypqhnAnW-TCQXa5jr-pIkUD_1DsfsPBUNkcxMTNZWjJFUUZOSFFROUVIMUVNVUVUSiQlQCN0PWcu\" target=\"_blank\" rel=\"noreferrer noopener\">Practicum Description Approval Form<\/a><\/p>\n\n\n\n<p>Before beginning your practicum, this form needs to be completed in consultation with your practicum supervisor.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Form Deadlines<\/h3>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Practicums Beginning in Fall Semester \u2013 September 1<\/li>\n\n\n\n<li>Practicums Beginning in Spring Semester \u2013 December 1<\/li>\n\n\n\n<li>Practicums Beginning in Summer Semester \u2013 April 30<\/li>\n<\/ul>\n\n\n\n<p class=\"fancyLink\"><a href=\"https:\/\/forms.office.com\/pages\/responsepage.aspx?id=2RNYUX1x3UWeypqhnAnW-TCQXa5jr-pIkUD_1DsfsPBUNEhTMFROUEVBQ1lMSkdRU0w5SVZUVlpXMiQlQCN0PWcu\" target=\"_blank\" rel=\"noreferrer noopener\">Practicum Student\u2019s Evaluation of Host Site Form<\/a><\/p>\n\n\n\n<p>Students will complete this form AFTER completing their practicum experience with the host site. This form is confidential and will not be shared with host sites.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Form Deadlines<\/h3>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Practicums Ending in Fall Semester \u2013 December 15<\/li>\n\n\n\n<li>Practicums Ending in Spring Semester \u2013 May 15<\/li>\n\n\n\n<li>Practicums Ending in Summer Semester \u2013 August 30<\/li>\n<\/ul>\n\n\n\n<p class=\"fancyLink\"><a href=\"https:\/\/forms.office.com\/pages\/responsepage.aspx?id=2RNYUX1x3UWeypqhnAnW-TCQXa5jr-pIkUD_1DsfsPBUREIyTDVIQVJBWjUwMVY4M0hZSE82ODVORCQlQCN0PWcu\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Practicum Supervisor\u2019s Evaluation Form<\/strong><\/a><\/p>\n\n\n\n<p>Each student is required to complete a minimum total of 270 hours of practicum for graduation. This is typically broken into two experiences; one at 90 hours and the other totaling 180 hours. The student receives academic credit for practicum and a grade of pass\/fail. The grade is based upon completion of the required hours, a presentation of their practicum work, completion of work in their practicum seminar, and your evaluation.<br><br>Thank you again for providing this opportunity. Because your evaluation is required for the student to receive their grade, please complete this form immediately after completion of the practicum<br><br><strong>NOTE: <\/strong>Because this practicum is a learning experience for the student, this evaluation is not confidential. This evaluation must be reviewed with the student prior to submission. The event occurs yearly in mid-September<br><br><strong>Practicum Approval Forms Due: <\/strong>For practicum to be completed in the spring semester, forms are due December 1. Approvals for summer practicum are due April 30, and approvals for fall practicum are due September 1.<br><br><strong>Practicum Evaluation Forms Due: <\/strong>For practicum completed in the spring semester, evaluation forms, time logs, and papers are due April 30. For practicum completed in the fall, evaluations are due November 30. For practicum completed in the summer, evaluations are due August 30.<br><br><strong>2nd Year Pecha Kuchas:<\/strong> A requirement for every second year grad to present on their practicum experiences. The event occurs yearly in early December.<br><br><strong>Transition Workshop:<\/strong> An event for second year grads to assist in the transition from part-time to full-time professional. It occurs yearly in late fall.<br><br><strong>Resume Book:<\/strong> Second year grads submit a resume to the SAHE program coordinator by January 15.
